Student Registration
Due to current social distancing guidelines, Skokie School District 73.5 has moved Pre-Registration, Residency Verification, and Registration online.
Please note you may be asked to submit additional documentation after everything has been reviewed.
Follow the 5 easy steps below to complete the registration process.

This form is STEP ONE to enroll your child as a NEW student at any of the schools in Skokie School District 73.5.
NOTE: Students moving from Meyer to Middleton or Middleton to McCracken are not considered "new students."
Please be prepared to scan and upload the following:
- your child's original birth certificate (must be a government-issued document)
- parent/guardian driver's license (with current, in-district address)
- a completed Home Language Survey (linked HERE)

This form is STEP TWO. As a parent or guardian of a student in Skokie School District 73.5, there are some requirements that must be updated or confirmed each year.
Please be prepared to scan and upload the following:
- Current mortgage statement OR most recent Real Estate Tax bill (if you own)
- Two different utility bills (if you own or rent)
- Current lease (if you rent)
IF you have more than one child attending, you only need to complete the form ONCE. Do not complete a separate form for each child.
Below are the Health Forms. Please click on a form to download and for easy viewing. These forms need to be submitted before the start of the school year.

Food Allergy, Asthma & Seizure Action Plans
ONLY for students that have an action plan
If transferring from a school within Illinois, students in all grades other than those listed will not need new forms provided up-to-date records are provided from the transferring school.
The PowerSchool Registration online system — called "e-Registration" — allows the district to collect and verify all student demographic, emergency, and medical information for your child(ren) prior to the start of the new school year. e-Registration will also allow you to copy shared family information, such as address and emergency contacts, to all other students in your family and electronically sign the annual permissions, photo and video releases, and device take-home agreements.
In early July, the Technology Department will send an email with instructions for using e-Registration for your student(s). The mailing will contain a link to the PowerSchool Parent Portal. You will log in to your Parent Portal account and submit an e-Registration form for each student in your family.
Families should take action and complete the e-Registration forms as soon as possible after receiving the email. e-Registration must be completed before a student can start school.
NEW for 2020-2021: Student Contacts Management!
We are excited to introduce a new Student Contacts management feature which allows parents/guardians to provide more information during e-Registration:
- Multiple contacts, their relationship to the student, and the order in which contacts should be called in an emergency
- More options for phone numbers and types, including identification of a preferred phone number
- Permission indicators for school pickup and emergency contact
- Additional mailing address for non-custodial parents/guardians
- "Custody" and "Lives With" indicators to help school staff better understand student living arrangements
