Meyer Student and Staff Flu Clinic

The seasonal flu vaccines will be administered by Advocate Children's Hospital staff on board the Ronald McDonald Care Mobile during the school day at Meyer and McCracken today and at Middleton on Tuesday, November 11th.

Parent/Guardian consent is required for children to participate. Please complete this Google Form ONCE PER student.

After submitting the Google Form, you will be able to download the consent form. Please fill out the consent form and return it to your school nurse. You will be notified by email to confirm your child's registration.
