Dear Parent/Guardian,

This letter will provide you with important information regarding the health and immunization requirements for your 6th grade student for the 2022-2023 school year. 

Paperwork

Illinois state law requires the following paperwork for all incoming 6th grade students:

  • Illinois Physical with up to date immunizations: Tdap and Meningitis (must be dated after August 25, 2021 to be valid)
  • Dental exam (must be dated after November 15, 2021 to be valid)

Forms are due to the health office by October 15, 2022. Per Illinois school code, students who do not have a current physical examination and immunization record on file will be excluded from school on Friday, October 15, 2022, and until such requirements are met. If you have not already done so, please make an appointment with your primary doctor as soon as possible to ensure that this requirement is met before the deadline. If you cannot obtain an appointment before October 15, 2022, please provide the health office with a note from your doctor’s office stating the date of your upcoming appointment.

Medications

All medications must be kept in the health office. With written permission, students may carry an inhaler, EpiPen, and/or medication for diabetes. All medication, including over the counter medication, requires a medication authorization form on file that is renewed every year.

Action Plans

If your child has a medical condition that requires an Emergency Action Plan (asthma, food allergy, seizure disorder, diabetes), please visit the Health Services page on the SD73.5 website to access the appropriate plan. Review the plan with your doctor and return it to the nurse’s office.

  • Drop off the forms at the McCracken main office
  • Fax forms to school at 847-673-1565, Attention: Nurse
  • Email forms to the nurse at [email protected]
