Dear Parent/Guardian,
Illinois School Code requires a dental examination to be completed for all students in the 6th grade. If you turned in a dental form for your student dated November 15, 2021, through May of 2023, then it is a valid dental examination form.
If you are unsure if your 6th grade student has a current dental examination form in their school health record, please contact Marryann Warda, R.N. at [email protected] or 847-676-8235.
The required Proof of School Dental Examination Form is linked for your convenience. Please return the completed form to school before the deadline of May 15, 2023. If you have a scheduled appointment after the deadline, please call the school health office with your appointment date.
