THIS JUST IN! The Pfizer Booster will be available at the January 8th vaccine clinic for children ages 12-15 who meet the following guidelines:

  • have received Dose #1 and Dose #2 of the COVID vaccine
    AND
  • at least 6 months have passed since Dose #2 was received

Registration is recommended to help Prism Health Lab plan for adequate supply of boosters.


COVID Booster appointments are still available for teens age 16 and older. Those who received COVID Vaccine Dose #1 at a December clinic can get their Dose #2 vaccine.

Children age 5 and older and adults can get their 1st COVID vaccine at the Middleton clinic. The 2nd vaccine in a series that starts on January 8th will need to be arranged elsewhere. Visit vaccines.gov to find a location within 10 miles of Skokie, or search by ZIP code for vaccine availability in other locations.

Services are provided by Prism Health Lab at Middleton Elementary SchoolRegister or walk in on Saturday January 8th from 8:00 a.m. to 12:00 p.m.
