5Essentials Parent Survey (through 2/15/19)

The Illinois 5Essentials Parent Survey will be conducted now through February 15, 2019. To take the survey please visit https://survey.5-essentials.org/Illinois and click the “Parent Survey” to begin. If you have children in multiple buildings, please complete one for each school.

Currently, 13% of parents district-wide have completed the survey. Please help us reach our goal of 20% participation!
