Thank you to families that have submitted their Dental Clinic consent forms for the upcoming in-school exams. We are accepting consent forms until Friday, February 23rd.
Miles of Smiles, Ltd., will be on site to provide dental exams during the school day on the following dates:
- March 8, 2024, at Middleton
- March 15, 2024, at Meyer
March 20, 2024, at McCrackenCANCELLED
All students are eligible, and there is no out-of-pocket cost for families or the school. This exam fulfills the Illinois requirement for a dental health examination for students in Kindergarten, 2nd grade, and 6th grade.
Participation is voluntary. If you would like your student to receive a dental exam, please complete this consent form as soon as possible and return it to your child's school nurse. Only students with a completed consent form can participate.
